COMPANY INFORMATION

Q: Who is the insurance company for the medical benefits?
A: We use various A.M. Best rated carriers for differt plans for different states

Q: What Preferred Provider Organization is offered through this plan?
A: Multiplan network. Multiplan offers one of the largest seamless national networks with over 580,000 providers in 50 states.

Q: Who is the TPA for the Select Series Nova plans that works with the Doctor and the PPO network for the client?
A: CONSOLIDATED HEALTH PLANS (CHP) in Massachusetts

GENERAL POLICY INFORMATION

Q: What are the effective dates?
A: On the 1st and the 15th of every month.

Q: What is the cutoff for the 1st of the month effective dates?
A: Applications must be received by 11:59 PM on the 15th of the previous month.

Q: When is the cutoff for the 15th of the month effective dates?
A: Applications must be received by 11:59 PM on the 1st of the month.

Q: Is there a “free look” provision with this plan?
A: Yes, there is a 10 day free look from the effective date.

Q: How long does an existing client have to add a newborn child to the policy?
A: 30 days.

Q: Are the policy benefits applied after the PPO discount?
A: Yes. The member will receive the Multiplan PPO discount first and then insurance benefits will apply.

Q: Are there any ineligible occupations for the plan?
A: No. Any occupation or industry is eligible for the plan.

Q: If a client has both a major medical and the plan how will the benefits be paid?
A: The plan benefits will be paid as described in the policy. There is no coordination of benefits.

Q: If a client cancels coverage will a certificate of credible coverage be issued?
A: Yes, a certificate will be automatically issued within 30 days after termination.

Q: Is there a maximum issue age for the stand alone Rx card?
A: Age 65

Q: Is there a lifetime maximum on the $5,000 or $10,000 Accident Riders?
A: No. There is no lifetime or annual maximum on the Accident Riders. There is no limit on how many times an insured’s family uses the policy.

Q: Can a client move from the Select 500 to the Select 1000 plan at any time?
A: Only at the policy renewal date on April 1st every year. The home office must approve this benefit.

Q: Can a client move from the Select 1000 plan to the Select 500 plan at any time to save premium?
A: Yes. They can move from a higher grade plan to a lower grade plan at any time, once in a 6 month period.

Q: What is an expected average discount for the Rx Card for BRAND name drugs for a walk in pharmacy?
A: 10%

Q: Does Urgent Care fall under the doctor visit benefits?
A: No. However there may be emergency room or urgent care benefit for the particular plan you are enrolled in.

Q: If a client with the Select 1000 Plan goes in for an annual exam and blood work what will they receive?
A: The client receives $100 Preventive Care Test Benefit.

Q: Does the Critical Illness Plan pay the benefit in a lump sum?
A: The insurance carrier will pay one time benefit of $10,000 for the diagnosis of a critical illness.

COMMISSIONS

Q: When are commissions paid?
A: 30 days after the policy effective date and renewal dates. Example: June 1 premium will be paid on June 30th.

Q: How often are commissions paid?
A: Commissions are paid to the agents on the 15th and the 30th, 30 days after the effective date or renewal date.

Q: Are the commissions level?
A: Yes, you as the licensed writing agent will be paid a level commission for as long as your client remains an active member with our plan.

Q: Is there any paperwork I need to fill out from NOVA to be appointed to sell these plans?
A: No. As an Agent, you will need to fill out AIM contracting paperwork. Your enrollment web page will not be active until AIM approves your contracting paperwork. Expect 10 business days for approval. NOTE: You must include copies of any state license that you intend to market or sell in.

BILLING

Q: When will the client be billed for their initial first premium payment when using EFT (Electronic Funds Transfer)?
A: For effective dates of the 15th of the month the premium payment date is the 15th of the month. For effective dates of the 1st of the month the premium payment date is the 30th of the month prior to effective date.

Q: When will the client be billed for their monthly renewal payments when using EFT?
A: For effective dates of the 15th of the month the premium payment date is the 10th of the month. For effective dates of the 1st of the month the premium payment date is the 20th of the month.

Q: When will the client be billed for their initial first premium payment when using credit cards payments?
A: For both the 1st and the 15th effective dates credit card payments are billed instantly upon enrollment.

Q: When will the client be billed for their monthly renewal payments when using credit card payments?
A: For effective dates of the 15th of the month the premium payment date is the 5th of the month. For effective dates of the 1st of the month the premium payment date is the 21st of the month.

Q: What is the minimum size list bill available?
A: List bill group’s with two or more members may apply for the Nova Plan.

Q: What is the list bill fee?
A: The list bill fee for all group sizes is $10.00 per month.

Q: Is there a grace period for groups?
A: Premiums are due no later than 10 days after the renewal date.

Q: Will AIM accept an Employer check for payment of premium?
A: Yes.
